<p><strong>Responsibilities</strong></p>

&#9;<li>Participate in clinical meetings, present updates to leadership, and maintain collaborative relationships with local systems such as DSS, DOE, Medicaid, and others. Managing transitional coordinator caseloads and supervising staff will be key aspects of your role, alongside ensuring compliance with state and federal guidelines.
&#9;<li>Travel to familiesâ homes and work at least one weekend day per week

<p><strong>Requirements</strong></p>

&#9;<li>Minimum of 2 years experience providing direct services or linking children with SED to supportive services

&#9;<li>Serve as a primary liaison with funders, regulatory agencies, and community partners to promote program visibility and compliance.
&#9;<li>Oversee grant and contract management, including timely reporting and achievement of performance indicators.
