Job Description
Our client, mission-driven, community-based hospice program that is setting the standard for compassionate end-of-life care for adults with terminal illness, is hiring a Director of Social Services, LCSW. This role is reserved for an experienced hospice social work leader. This is a critical leadership role reporting directly to the Hospice Administrator and partnering closely with the Nurse Manager to support a growing interdisciplinary team. The ideal candidate will be either a current Director or strong Manager within a hospice setting and must be a Licensed Clinical Social Worker (LCSW) in New York.
Responsibilities:

Provide clinical oversight and supervision to a team of LMSWs.

Maintain a personal caseload of 27–35 patients, all seen in person.
-
Conduct case reviews in partnership with the Nurse Manager and contribute to care planning, quality initiatives, and regulatory compliance.
-
Participate in admin on-call rotation alongside other clinical leaders.
-
Collaborate on policy development and workflow improvements to support clinical excellence.
-
Rotate into the Bronx home base 2–4 times per month, typically for 4–5 hours in the morning.

Ensure seamless coordination and communication across teams delivering care in the field.
Qualifications:

LCSW required with active New York licensure.

Minimum 5 years of progressive experience in hospice social work, including recent supervisory or leadership roles.

Strong understanding of hospice regulations, psychosocial dynamics of end-of-life care, and interdisciplinary collaboration.

Proven ability to manage caseloads while providing clinical leadership to a diverse team.
- Experience in a community-based hospice setting required.
