Job Description
Our client, a Federally Qualified Health Center (FQHC) with multiple locations in New York City, is hiring a Psychiatric Nurse Practitioner. If you are a compassionate and skilled Psychiatric Nurse Practitioner, join our team in providing high-quality, patient-centered care to individuals living with severe mental illness. As a Psychiatric Nurse Practitioner, you will play a critical role in managing psychiatric care, prescribing medications, and providing therapeutic interventions in a collaborative, multidisciplinary team environment. This role will focus on integrated care, working closely with multidisciplinary teams to ensure members have rapid access to psychiatric care and comprehensive support, including therapy, primary care, and specialty services. The PNP will play a critical role in managing psychiatric care, prescribing medications, and providing therapeutic interventions for individuals with complex needs. This position offers the opportunity to work in a collaborative, mission-driven environment that places a strong emphasis on integrated, community-based mental healthcare.
Responsibilities:
- Conduct psychiatric evaluations and comprehensive assessments for members.
- Diagnose mental health conditions and create individualized treatment plans that include medication management and therapeutic interventions.
- Prescribe, adjust, and monitor psychiatric medications, including but not limited to antidepressants, antipsychotics, and mood stabilizers.
- Provide ongoing psychiatric care, ensuring the continuity of care for individuals with serious mental illness.
- Collaborate closely with primary care providers, social workers, and other specialists to ensure holistic care for each patient.
- Develop and maintain comprehensive patient records in accordance with organizational standards and regulations.
- Participate in weekly multidisciplinary case reviews to discuss patient progress and adjust care plans as necessary.
- Offer psychoeducation to patients and their families, helping them understand their conditions and treatment options.
- Engage in care coordination efforts to ensure that patients receive timely services and are connected to appropriate community resources.
- Support the broader mission by promoting the integration of mental health services with social support, focusing on the holistic care of individuals with severe mental illness.
Qualifications:
- Master’s degree in Nursing (MSN) with a focus in Psychiatric/Mental Health Nursing.
- Current, active New York State Psychiatric Nurse Practitioner license (NP) with board certification in psychiatric mental health.
- DEA Controlled Substance Registration Certificate required
- At least 2 years of experience in psychiatric care, preferably in community-based settings or working with individuals with serious mental illness
- Strong clinical skills in psychiatric assessment, diagnosis, and medication management.
- Ability to work effectively in a collaborative, multidisciplinary team environment.
- Experience with community-based mental healthcare and a commitment to serving underserved populations.
- Familiarity with mental health conditions, including mood disorders, schizophrenia, anxiety disorders, and trauma-related conditions.
- Excellent communication skills, including the ability to build rapport with patients and their families.
- Compassionate, patient-centered approach to care, with a strong commitment to improving the quality of life for individuals with severe mental illness.

- Bilingual English / Spanish a plus!
- Proficiency in use of electronic medical records, preferably eClinicalWorks (eCW) software
