Job Description
Our client, a community health center, is hiring a Population Health Manager. This is a strategic leadership role for someone passionate about data-driven healthcare transformation and committed to advancing equity and quality. The Population Health Manager is responsible for leading organization-wide strategies to improve clinical outcomes, patient access, and overall population health performance. This role leverages data analytics, performance metrics, and value-based care principles to drive improvements in quality, efficiency, and patient-centered care.
Key Responsibilities:

Lead data-informed initiatives to enhance population health outcomes and reduce disparities.

Monitor clinical quality metrics (HEDIS, QARR, UDS), identify care gaps, and support alignment with national and state benchmarks.
-
Oversee quality improvement and risk management programs using tools like Plan-Do-Study-Act (PDSA).
-
Manage pay-for-performance and shared savings programs, including contract oversight and performance reporting.
-
Provide strategic oversight of patient attribution, panel management, and incentive program participation.
-
Chair or co-chair internal committees focused on quality, risk, and population health.
-
Supervise staff and ensure coordination across departments for training, compliance, and clinical performance.
-
Support operational oversight of Health Home programs through September 2025.
Qualifications:
-
Bachelor’s degree in public health, healthcare administration, or related field (Master’s preferred).
-
5–10 years of progressive healthcare management experience, including supervisory leadership.
-
Expertise in health data analytics, quality improvement, and population health tools.
-
Strong knowledge of value-based care models and associated metrics (HEDIS, QARR, P4P, UDS).
-
Excellent communication, project management, and cross-functional leadership skills.
-
Proficient in Microsoft Office, particularly Excel, and reporting platforms.
