Join a Leading Rehabilitation Healthcare Provider as a Outpatient Physical Therapist. Our client, a respected non-profit acute rehabilitation hospital and member of a major health system is hiring a Physical Therapist for outpatient services. Responsibilities:

  • Deliver individualized outpatient rehabilitation plans focusing on orthopedic and neurological conditions, including post-op orthopedic cases, stroke, TBI, and vestibular disorders.
  • Conduct 45-minute one-on-one therapy sessions, ensuring optimal patient engagement and progress.
  • Collaborate with a multidisciplinary team comprising PTs, OTs, Speech Therapists, and other healthcare professionals to develop and implement comprehensive treatment strategies.
  • Utilize the Epic EMR system accurately to document patient care and progress.
  • Participate in ongoing professional development, mentorship programs, and clinical education initiatives.
  • Maintain a patient-centered approach, ensuring safety, comfort, and high-quality care.
  • Support clinic operations, including flexible scheduling to meet patient needs, and engage in quality improvement activities.

Requirements:

  • Minimum of 1 year of outpatient physical therapy experience preferred.
  • Demonstrable commitment to a patient-first, quality-driven care model.
  • Proficiency with Epic EMR or similar electronic health records systems.
  • Strong clinical reasoning, communication, and teamwork skills.
  • Ability to adapt to a flexible schedule, including potential evening or weekend hours.
  • Current NY State Physical Therapy license.
