Job Description
Our client, a leading non-profit organization providing support to individuals with developmental, intellectual, and behavioral challenges in Westchester County, is currently seeking experienced Nurse Practitioners with a background in developmental disabilities to join their team. Under the direction of the Medical Director, the Nurse Practitioner will be responsible for developing and implementing medical care plans for individuals, in collaboration with the Individual Treatment Team (ITT) and the Individual Program Plan (IPP).
Responsibilities:
- Conduct physical exams, including basic screenings for vision and hearing, and document findings.
- Write medical orders and prescriptions for medications, devices, and immunizations in accordance with medical protocols.
- Communicate with parents/guardians regarding medical emergencies or significant changes.
- Collaborate with rehabilitation staff to evaluate and monitor individualized habilitation programs.
- Document relevant information and review related documents.
- Coordinate with healthcare professionals at acute care facilities.
- Conduct initial exams for new admissions and provide updates on medical information.
- Order diagnostic tests as needed and make diagnoses and treatment plans.
- Monitor and adjust medications, including seizure control.
- Perform discharge physical exams and provide recommendations for new placements.
Requirements:
- Valid Nurse Practitioner license with experience in developmental disabilities or related fields.
- Strong communication skills with the ability to work in a multidisciplinary team.
- Knowledge of clinical protocols for individuals with developmental, intellectual, and behavioral challenges.
- Graduate of an accredited professional nursing program (preference for Family Practice, Pediatrics, or Rehabilitation Nursing).
- Current NP license in New York State with certification through ANCC or AANP.
- Valid certifications in CPR, Infection Control, and DEA.
- At least one year of experience working with individuals with developmental disabilities.
- Proficiency in Electronic Medical Records (EMR) and e-prescribing platforms.
- COVID-19 vaccination is mandatory for all employees.
