Job Description

Our client, a premier patient-focused integrated healthcare delivery platform providing the highest level of quality care, is hiring a Nurse Practitioner. Join a dynamic and compassionate home care services team. In this vital role, the Nurse Practitioner will provide high-quality, patient-centered care to individuals in their homes, ensuring continuity of care and improved health outcomes.

Responsibilities:

Diagnosing and managing acute and chronic conditions: Assess patient health, develop personalized treatment plans, and provide ongoing management of various medical conditions.
Ordering and interpreting diagnostic tests: Leverage diagnostic tools to accurately evaluate patient needs and guide treatment decisions.
Prescribing medications: Utilize clinical expertise to prescribe appropriate medications tailored to each patient's condition.
Patient and family education: Offer comprehensive guidance on managing health conditions, disease prevention strategies, and treatment options to empower patients and their families.
Monitoring progress and outcomes: Track patient recovery, adjust treatment plans as needed, and ensure optimal care delivery throughout the course of treatment.
Job Requirements:

Master’s degree required.
Graduate of a Nurse Practitioner education program registered by the New York State Education Department.
Must possess a valid NYS license and DEA registration.
