Description: Hiring a Nurse Practitioner, Primary Care for a community health center dedicated to helping the underserved. The Nurse Practitioner, Primary Care, under the supervision of the Medical Director, is responsible for the provision of comprehensive high-quality care to new and existing patients. The Nurse Practitioner, Primary Care will collaborate with an interdisciplinary team and have experience caring for pediatric, adolescent and adult high risk and underserved patients.
Responsibilities:
  • Provides Primary Care, routine and acute care
  • Performs routine diagnostic and therapeutic procedures
  • Initiates treatment and management of chronic illnesses including, but not limited to, HIV infection, asthma, bronchitis, bronchitis, diabetes, lipid disorders, and
  • Generates appropriate referrals to ancillary and specialty services including medical, mental health and social based
  • Maintains documentation and e-prescribing practices according to standards of the agency and regulatory agencies including but not limited to history and physical, social history and literacy level as well as care
  • Ensure timely prescribing, orders, addressing labs, interpreting of test results, addressing telephone encounters, ordering and reviewing of specialty referrals
  • Provides health education and counseling during
  • Provides on-call telephone coverage on a rotating
  • Demonstrates proficiency of electronic medical record; including documentation, e-prescribing and
  • Participate in meetings including Quality of Care, Clinical Training, Business, Peer Review and Journal Club meetings

Education Requirements:
  • Graduate of Accredited Nurse Practitioner Program and possess advanced nursing degree.
  • Licensed and Registered in New York State
  • Certified by either the American Academy Nurse Practitioners or BLS Certified

Experience Requirements:
  • Experience preferred in treating hard to reach populations, including the homeless, immigrants, HIV infected, substance users, and mentally

Desired Skills:
  • Bilingual preferred (English/Spanish or English/French)
