Job Description

Our client, a leading community-based healthcare provider committed to delivering exceptional patient care, is hiring a Nurse Practitioner - Internal Medicine, Primary Care. This full-time position offers an excellent opportunity for motivated clinicians to make a meaningful impact in a collaborative, outpatient environment. Provide high-quality, patient-centered primary care in a supportive setting, working closely with physicians and healthcare teams to manage a diverse patient population. You will perform comprehensive health assessments, develop individualized treatment plans, and coordinate care across departments. Your role will involve managing chronic conditions, ordering and reviewing diagnostic tests, prescribing medications, and participating in hospital rounds when necessary. With an emphasis on teamwork and continuous quality improvement, you will contribute to enhancing patient outcomes and overall practice efficiency.

Responsibilities:

  • Collaborate with physicians and healthcare professionals to deliver comprehensive primary care services.
  • Conduct physical examinations, health assessments, and preventive screenings appropriate to patient age and history.
  • Develop and implement treatment plans, including prescribing medications and interventions.
  • Order, interpret, and review diagnostic tests and laboratory results within established protocols.
  • Facilitate patient education and coordinate referrals to specialists and community resources.
  • Participate in hospital rounds for new consults and follow-up care, including discharge planning.
  • Maintain accurate, timely documentation within the EMR system and ensure proper coding of patient visits.
  • Engage in quality assurance activities and continuous performance improvement initiatives.
  • Uphold patient confidentiality and demonstrate professionalism and respect in all interactions.

Requirements:

  • Graduate of an accredited Nurse Practitioner Program and licensed in New York State.
  • 2–5 years of related clinical experience or an equivalent combination of education and experience.
  • Demonstrated proficiency in advanced health assessment and decision-making.
  • Strong organizational and communication skills, with the ability to work effectively under stress.
  • Flexibility and readiness to adapt to changing clinical needs.
  • Commitment to providing respectful, compassionate patient care.
