Description: Hiring a Nurse Practitioner, Glycemic Control, for a top health center in Brooklyn.  Seeking a Nurse Practitioner, Glycemic Control, who is an expert in inpatient diabetes care who will contribute to excellence in patient care, teaching, and provide leadership. Responsible for the delivery of inpatient diabetes care and education that involves compliance with evidence-based clinical standards, outcomes measurement (tracking, analysis and reporting of clinical, financial, and satisfaction outcomes). Leads and coordinates activities to achieve and maintain JCAHO certification in diabetes care. As a member of the inpatient glycemic management team provides direct patient care and works collaboratively with the medical team to establish priorities and to assess solutions for patients to ensure they receive the best care possible.

Responsibilities:
  • Ensures compliance with regulatory requirements concerning glycemic management and diabetes education
  • Collaborates and works closely with Physician, nursing, pharmacy, and other departments to assess current processes, quality of care and barriers to necessary changes in clinical practice related to inpatient diabetes care.
  • Recommends equipment and products that are safe, effective and appropriate.
  • Collaborates with physicians and other educators in planning, developing and implementing in-services diabetes training sessions for staff
  • Communicates with patients and families clarify treatment plan
  • Represents hospital in community activities related to area of specialization.
  • Ensures discharge planning starts from admission and helps the medical and nursing staff facilitate patient self-care and prevent readmission.
  • Prepares a variety of reports that summarizes activities and outcomes of glycemic management and education for presentation to appropriate committees.
  • Assist as needed with the Quality Department in investigating patient incidents related to glycemic management.
  • Integrates appropriate research findings and best practices into practice.
  • Develops and implements studies relevant to clinical nursing issues in collaboration with other health care professionals.
  • Coordinates with Case Managers, Social Workers & Medical Doctors regarding discharge

Educational Requirements:
  • Graduate from an accredited school of nursing - MSN required
  • NY State RN license
  • NY State NP License and certification
  • NP Diabetes Certification preferred

Experience Requirements:
  • 3-5 years acute care nursing experience in diabetes management and education
  • Excellent oral and written skill. Able to multi-task and prioritize assignments independently.

Desired Skills:
  • Able to work collaboratively with fellow physicians and other health care providers.
