Description: Our client, a not-for-profit teaching hospital serving an ethnically diverse residential neighborhood of New York City, is hiring a Geriatrician or Palliative Care Physician.
Job Responsibilities:
- Join a faculty practice as a Geriatric clinician-educator
- Work closely with the Chair of the department to play a critical role in both care delivery, resident and medical student education and ongoing practice innovation
- Work with a team that will support you in delivering high quality patient care
- Provide best in class comprehensive primary care in an outpatient setting, PCMH
- Partner with transition care team; coaching and integrated behavioral health models to engage our geriatric patient population in care and drive behavior change.
- Help establish a Geriatric patient panel
- Embrace the opportunity to lead while working with a true team
- Utilize our Electronic Health Record to plan care for individual patients and maximize impact on the population
- Be a part of a practice-setting designed for a team-based practice model
Job Requirements:
- Board certification in Internal Medicine, and Geriatrics required.
- Spanish language skills desirable
- Currently licensed in the state of New York
- The ability to build successful relationships with team members and communicate effectively both 1-on-1 and in groups
