Description: Hiring a Family Nurse Practitioner, Substance Abuse for a community health center. The organization has a wide-ranging focus in creating healthy communities, prevention and wellness programs and the treatment of diseases. The Family Nurse Practitioner, Substance Abuse will Provide high quality primary care to pediatric and adult groups. The Family Nurse Practitioner, Substance Abuse must have experience with Substance abuse patients.

Responsibilities:

  • Provide immediate patient care and, where necessary, refer the patient to a primary care team for more comprehensive care
  • Develop and implement a plan of care in collaboration with other members of the care team, the patient, family members and significant others
  • Ensure high quality patient care is delivered cost-effectively, consistently
  • Provide appropriate specialist referrals, follow-up and hospitalization when indicated
  • Maintain electronic medical records
  • Maintain familiarity with ongoing research and development in relevant fields
  • Ensure performance improvement initiatives are supported with timely and accurate data and consistent actions

Education Requirements:

  • New York State Nurse Practitioner License
  • Bachelor’s Degree in Nursing
  • Master’s Degree in Nursing

Experience Requirements:

  • Must have experience with Substance abuse patients.
  • Minimum of one-year relevant experience; experience working in emergency, urgent care or walk-in clinic desirable

Desired Skills:

  • Bilingual: English and Spanish preferred
  • Outstanding “bedside manners”: patient-centric with empathy, excellent active listening and communication skills, engaging and efficient
  • Outstanding clinical capabilities and judgment
  • Responsive and capable of working in a high accountability environment
  • Comfortable using an electronic health record
  • Excellent judgment and ability to prioritize resources where they will be most useful
  • Comfortable receiving and providing constructive feedback
  • Engaged in learning and continuous quality improvement
  • Outstanding written and verbal communications skills
  • Passionate about providing high quality primary health care to those lacking access to health care
