Description: Hiring a Family Nurse Practitioner, School Health (FNP) for a top academic medical center. The Family Nurse Practitioner, School Health (FNP) will be responsible for practicing as a nurse practitioner consistent with what is considered standard primary care practice in general pediatrics and/or family medicine, including family planning. The Family Nurse Practitioner, School Health (FNP) will ensure high quality primary care is delivered and will be onsite at various clinics at various schools.


  • Provide comprehensive patient care and establish a family/school partnership to meet the needs of the patients on an ongoing basis; identify diagnoses and treat actual or potential health problems through case-finding, health-teaching and health counseling
  • Ensure high quality primary care is delivered cost-effectively, consistently
  • Provide appropriate specialist referrals, follow-up and hospitalization when indicated
  • Collaborate with health team to facilitate the highest quality of services
  • Evaluate patients and prescribe medications, treatments, referrals
  • Maintain electronic medical records in compliance with standards with high efficiency
  • Engage patient, family and cross-functional care teams in sustained health and well-being of the patient
  • Provide medical services to patients in accordance with network clinical policies
  • Participate in patient education and counseling services, especially about family planning, STD?S/HIV, preventive health and nutrition

Educational Requirements:

  • Current NYS license as a Nurse Practitioner (NP)
  • Master's Degree in Nursing

Experience Requirements:

  • Family Nurse Practitioner (FNP) with pediatric experience or Pediatric Nurse Practitioner
  • Minimum 1 year experience in Primary Care in an outpatient setting required
  • Outpatient experience required
  • Primary care experience required
  • Experience working in an under-served community highly preferred
  • Spanish or Chinese speaking preferred
  • Comfortable speaking to parents, principals and administrative staff
  • Strong interpersonal skills
  • No new grads
