Job Description
Our client, a not-for-profit teaching hospital serving an ethnically diverse community in New York City, is hiring a dedicated and compassionate Adult Nurse Practitioner (ANP) to join an interdisciplinary team. The ANP will deliver high quality, patient centered care to adults in an ambulatory setting, focusing on prevention, diagnosis, treatment, and management of acute and chronic conditions. This is an excellent opportunity to work in a collaborative environment that values innovation, clinical excellence, and patient engagement.
Key Responsibilities:
- Conduct comprehensive health assessments, including patient history, physical exams, and diagnostic testing
- Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals
- Manage chronic diseases such as diabetes, hypertension, and COPD, ensuring evidence based treatment and continuity of care
- Educate patients and families on disease prevention, health maintenance, and treatment options
- Order, perform, and interpret diagnostic tests and procedures as appropriate
- Document all patient interactions accurately and timely in the EMR
- Participate in quality improvement initiatives and interdisciplinary team meetings
- Ensure compliance with all regulatory and institutional policies and guidelines
Qualifications:
- Master’s Degree in Nursing from an accredited program
- Current licensure as a Nurse Practitioner in New York State
- Certification as an Adult Nurse Practitioner (ANCC or AANP)
- DEA registration and BLS/ACLS certification
- Minimum of 2 years of clinical experience preferred, though new graduates with strong clinical rotations may be considered
- Commitment to patient-centered, culturally competent care
